NHS Blood and Transplant does not exclude smokers from donation

Smoking does not disqualify you from donating blood in the UK. NHS Blood and Transplant accepts blood from smokers without requiring any waiting period or abstinence before donation. There is no restriction on donating if you have smoked before your appointment. However, smoking significantly affects your body over time, including your cardiovascular system and haemoglobin levels and in some cases the effects of long-term heavy smoking may affect whether donation proceeds on the day of your appointment, based on individual health assessment.

HAEMO GLOBIN

Haemoglobin Levels

Blood donors must meet a minimum haemoglobin level before donating. Smoking affects red blood cell production and oxygen delivery through carbon monoxide binding to haemoglobin, which can in some cases reduce effective haemoglobin function. The NHS tests haemoglobin at every donation appointment and will defer donation if the level is too low. This is not specific to smokers but long-term heavy smokers may be more likely to encounter this issue.

HEALTH ASSESSMENT

The Health Check Process

Before every donation, you complete a health questionnaire and a brief physical check. Blood pressure, pulse, haemoglobin and general health are assessed. Any health condition that affects your blood safety or your ability to safely lose a unit of blood may result in deferral on that occasion, regardless of smoking status. The assessment is individualised.

VAPING

What About Vaping?

Vaping does not appear on NHS Blood and Transplant's standard deferral list as a specific exclusion for nicotine-containing products. If you have switched from cigarettes to vaping as part of a cessation journey, this does not in itself affect your eligibility to donate. Always declare your full current health status at the appointment.
