Your mouth is the primary entry point for vapour and it is where vaping's effects are most directly felt. Here is a comprehensive overview of the oral health picture for vapers, what the risks are, what they are not and what you can do.
Vaping affects oral health through four main mechanisms: PG-induced dry mouth reducing saliva's protective functions, nicotine's vasoconstrictive effect on gum blood supply masking early gum disease, potential staining from nicotine deposits, and chronic mild airway and oral tissue irritation. These effects are real and warrant attention and proactive oral hygiene. However they are substantially less severe than the oral health impacts of tobacco smoking, which carries a dramatically higher oral cancer risk, causes more severe gum disease, produces worse staining from tar, and produces deeper dental destruction over time.
Propylene glycol draws moisture from oral tissues, reducing salivary flow. Saliva is the mouth's primary defence: it washes away food and bacteria, buffers acids, and contains antimicrobial proteins. Reduced saliva allows plaque-forming bacteria to accumulate more freely, increases acid attack on enamel and creates the conditions for gum disease to develop. Dry mouth is the most consistently significant oral health risk from vaping and the one most easily mitigated through hydration and higher-VG liquids.
Nicotine's vasoconstrictive effect reduces blood flow to gum tissue and suppresses the inflammatory response that normally signals gum disease through bleeding on brushing. This means vapers may develop gum disease without the usual early warning signs, by the time symptoms become apparent, the condition may be more advanced than it would be in a non-vaper who experienced the normal early indicators. Regular dental checkups are more important for vapers for this specific reason.
Nicotine deposits on enamel produce yellowish staining over time. This is lighter and more superficial than the tar-based staining from cigarette smoking and responds better to dental cleaning and whitening treatments. Vapers who were previously smokers typically notice significantly improved tooth colour after switching, even without whitening treatment.
Tobacco smoking dramatically increases oral cancer risk through combustion carcinogens that contact the oral mucosa with every puff. Vaping does not produce these combustion carcinogens. While no form of oral exposure is entirely risk-free, the oral cancer risk associated with vaping is expected to be substantially lower than smoking, though longer-term data is still developing.
Tell your dentist you vape so they can monitor specifically for early gum changes that your suppressed inflammatory response may not signal. This is the single most important step.
Brush twice daily at the gumline, use interdental brushes or floss daily and use alcohol-free antibacterial mouthwash. With reduced saliva, this routine needs to be more thorough than for a non-vaper.
Consistent water intake throughout the day offsets PG-induced dry mouth and supports saliva's protective functions.
Reducing PG content reduces dry mouth and its downstream effects on plaque and gum health.
